
Social Health Engagement Specialist
GENERAL RESPONSIBILITIES:
Identifies social determinants of health within the mobile health unit, Whitney on Wheels (WOW) population to link them to appropriate resources. The Social Health Engagement Specialist will coordinate and collaborate with the WOW Team as well as external partners to engage eligible program participants and ensure that existing patients’ social and ancillary needs are being met.
SPECIFIC RESPONSIBILITIES:
- Conduct social determinant of health screenings on all eligible patients that utilize WOW.
- Initiate, track, and complete referrals for any patients in need of service coordination.
- Implement and monitor a tracking system for qualitative and quantitative measures such as number of referrals generated, number of referrals completed, and patient testimonials.
- Schedule, facilitate, and participate in performance improvement activities including a biannual meeting with WOW partner sites.
- Attend regular training to remain current with best practices, new resources, and opportunities to better serve WOW patients.
- Maintain timely, accurate documentation of communication, referrals, supplies, inventories and all other related paperwork.
- Collaborate with site, WOW Team, and Director of Community Based Initiatives and Program Support to ensure patient care needs are met.
- Provide presentations for the Community Based Programs Team and other WYH staff.
- Completes tasks according to grant-defined work plan prior to deadlines.
- Demonstrated time management and multi-tasking skills.
- Demonstrates excellence in both internal and external customer service.
- Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality.
- Ensures and/or remains in compliance with local, state, and federal regulation, i.e. DHHS HRSA and NYSDOH, and all accreditation standards (e.g. Joint Commission and NCQA-PCMH).
- Adheres to the National Patient Safety Goals as defined by the Joint Commission and Whitney M. Young Jr. Health Center.
- Completes other duties as assigned.
MINIMUM QUALIFICATIONS:
Bachelor’s Degree in a health-related field. Two (2)+ years’ experience working with diverse populations; Effective verbal, written, and public speaking skills; Experience working and engaging community partners; Strong organizational skills with knowledge of Excel and data tracking.
PREFERRED QUALIFICATIONS:
Master’s Degree in Public Health or health-related field. Bilingual (English/Spanish); Experience working or living in the target community and knowledge of community resources.

Salary range: $20.00 - $22.00 hourly
