  • Personal Care Assistants (PCA) / Home Health Aides (HHA)
  • Licensed Practical Nurses (LPN)

We are hiring in the Capital Region and all surrounding counties: Saratoga, Columbia, Rensselaer, Albany, and Schenectady counties. Compensation is based on role and experience.

Personal Care Assistants (PCA) / Home Health Aides (HHA)

Job Overview: As a PCA or HHA, you will assist individuals with daily living activities, such as personal hygiene, grooming, mobility assistance, and meal preparation. Your role is essential in supporting clients in their homes, ensuring their comfort and safety.

Key Responsibilities:

  • Assist with personal hygiene (bathing, dressing, grooming)
  • Help with mobility and transfers
  • Assist with meal preparation and feeding
  • Provide companionship and emotional support
  • Monitor and record client health conditions
  • Ensure safety and comfort in the home setting

Qualifications:

  • New York State PCA/HHA certification (required)
  • Compassionate, patient, and empathetic demeanor
  • Ability to follow care plans and report changes in patient status
  • Prior experience in home care setting preferred but not required

Licensed Practical Nurses (LPN)

Job Overview: As an LPN, you will provide nursing care to patients under the supervision of a registered nurse or physician. You will monitor vital signs, administer medications, assist with treatments, and offer emotional support to clients and their families.

Key Responsibilities:

  • Monitor and record patient vitals and health conditions
  • Administer prescribed medications and treatments
  • Assist RNs and physicians in patient care procedures
  • Provide wound care, catheter care, and other specialized services
  • Offer education to patients and families regarding care plans
  • Maintain accurate medical records and documentation

Qualifications:

  • New York State LPN license (required)
  • Excellent communication and interpersonal skills
  • Ability to work in a team environment
  • Previous experience in home care setting preferred but not required

Registered Nurses (RN)

Job Overview: As an RN, you will take a leadership role in coordinating and delivering patient care. You will assess, plan, and implement nursing care, supervise LPNs, PCAs, and HHAs, and communicate with physicians and families to ensure high-quality care for patients.

Key Responsibilities:

  • Assess patient health and develop individualized care plans
  • Administer medications and treatments as prescribed
  • Monitor patient progress and adjust care plans as necessary
  • Supervise and provide guidance to LPNs and nursing assistants
  • Communicate with patients, families, and interdisciplinary team members
  • Provide patient education and emotional support

Qualifications:

  • New York State RN license (required)

Occupational Therapists (OT)

Physical Therapists (PT)

  • Educate patients on proper movement techniques to prevent injury

Qualifications:

Office Assistant

Job Overview: As an Office Assistant, you will provide essential administrative and operational support to ensure the smooth delivery of care services to clients in their homes. Your role will involve managing office tasks, coordinating schedules, maintaining client records, and assisting with communication between caregivers, clients, and healthcare providers. This position is critical in supporting the agency’s mission to provide high-quality, compassionate care to individuals in need.

Key Responsibilities:

  • Handle general office tasks, including answering phones, scheduling appointments, and responding to emails.
  • Manage the daily operations of the office, ensuring that documentation and records are accurate and up to date.
  • Prepare and organize client files, ensuring all necessary paperwork and consent forms are completed and filed properly.
  • Schedule and coordinate caregiver shifts, ensuring the right staff is assigned to the right clients based on availability and care needs.
  • Communicate effectively with caregivers, clients, and family members regarding schedules, changes, and updates.
  • Serve as a point of contact for clients, caregivers, and healthcare professionals.
  • Address client inquiries and concerns in a timely, professional, and compassionate manner.

Qualifications:

  • Previous experience in an administrative role for 1 year, preferably in a healthcare or home care setting.
  • Strong organizational skills with the ability to manage multiple tasks simultaneously.
  • Excellent written and verbal communication skills.
  • Attention to detail and accuracy in managing records, schedules, and billing.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook) and comfortable using office equipment.
