
RN, LPN, PCA, HHA, PT, OT, SW and Office Assistant
Overview
We are seeking a compassionate and dedicated PCA/HHA/LPN/RN, SW and Occupation Therapist/Physical Therapist to provide essential support to individuals in need of assistance with daily living activities. This role is vital in enhancing the quality of life for our clients, ensuring they receive the care and attention necessary to maintain their independence and dignity. The ideal candidate will possess a strong commitment to providing personalized care in a respectful and empathetic manner.
Responsibilities
- Assist clients with personal hygiene, grooming, and dressing.
- Provide meal preparation and ensure clients receive nutritious meals.
- Assess, plan, implement, and evaluate patient care needs.
- Administer medications and treatments as prescribed by physicians.
- Monitor and record patient vital signs and progress.
- Collaborate with interdisciplinary teams to develop and update care plans.
- Educate patients and families about their health conditions and care plans.
- Provide emotional support and comfort to patients and their families.
- Ensure compliance with all hospital policies, procedures, and standards of care.
- Participate in quality improvement initiatives and continuing education.
- Maintain a clean and safe living environment for clients.
- Offer companionship and engage clients in meaningful activities.
- Assist with mobility and physical support as needed.
Experience
- Previous experience in assisted living or personal care home settings is preferred.
- Familiarity with medication administration and first aid practices is advantageous.
- Current and valid Nursing license in New York State.
- Strong clinical knowledge and skills in patient care.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced, dynamic environment.
- Strong communication skills and the ability to build rapport with clients are essential.
- A valid driver’s license may be required for transportation duties.
